Missed Call Text Back for Med Spas: Templates, Setup, and the Rules
By Eric Dunn (CEO & Co-Founder, Aesthetix CRM)

The phone still matters in aesthetics. A patient who calls is usually further along than one who fills out a form: they have a question, a date in mind, or they are ready to book. And the phone rings at the worst possible moments, while the front desk is checking someone in, processing a payment, or already on another line.
Many callers who reach voicemail hang up without leaving a message. Then they call the next practice on the list.
Missed call text back fixes that with one automated text. This guide covers how it works, what the message should say, how to set it up properly, and the rules that keep it compliant.
What Is Missed Call Text Back?
Missed call text back is an automation that sends a text message to a caller when their call goes unanswered. Within seconds of the missed call, the caller gets a message from your practice’s number acknowledging the call and inviting them to continue the conversation by text.
It turns a dead end (a ringing phone, a voicemail greeting, a hang-up) into an open conversation, on the channel most patients prefer anyway.
Why It Matters for Aesthetic Practices
- Callers are high-intent leads. Someone calling about a consultation is often ready to act today. Losing that call costs more than losing a casual web visitor.
- Many will not leave a voicemail. A missed call without a message leaves you with a number and no context, and calling back an hour later often reaches voicemail on their end.
- Speed decides who gets the consult. Research on speed to lead is consistent: the first practice to respond usually wins. A text within seconds of the missed call puts you first.
- Many calls come after hours. Patients call on lunch breaks, in the evening, and on weekends. A text back is often the only response that arrives while they are still thinking about the treatment.
- Patients would rather text. Plenty of patients find it easier to ask about pricing or downtime by text than to play phone tag.
How Missed Call Text Back Works
- A call comes in and rings. Your phone system rings your team for a set time, usually 10 to 20 seconds.
- The call is classified as missed. If nobody answers within that window, the call goes to voicemail and is marked missed.
- The text sends. An automation sends the caller a text from the number they called, personalized with your practice name and, if the caller is already in your CRM, their first name.
- A callback task is created. The automation creates a task for the right team member to call back or reply, so a person follows up.
- The conversation continues. If the caller replies, the thread lands in your inbox on their contact record, with the missed call and any voicemail attached.
The best setups add two controls: a wait period so a caller who tries three times in five minutes gets one text, not three, and separate messages for business hours and after hours.
Missed Call Text Back Templates
Keep the first text short, personal, and useful. It should tell the caller who you are, acknowledge the call, and give them an easy next step. Replace the bracketed fields with your own merge fields.

Consultation line (for a number used on consult ads)
Hi, this is [Practice Name]. Sorry we missed your call about a consultation. We’d love to help. What’s the best time to reach you today? You can also reply here with any questions.
Spanish-language line
Hola, le saluda [Practice Name]. Lamentamos no haber contestado su llamada. Responda a este mensaje y le ayudaremos pronto.
A few things the templates deliberately leave out: discounts, specials, and promotions (see the rules below), and anything about a specific treatment or appointment for a caller you have not identified. Keep it to an acknowledgment and a next step.
The Rules: TCPA, Carriers, and Privacy
Missed call text back is one of the safer automations in your stack, provided it stays a reply rather than a marketing message. This is general information, not legal advice; your counsel should review your specific setup.
- Keep the first text informational. A one-time text responding to a call the person placed is generally treated as an informational reply covered by the caller’s prior express consent under the TCPA rules at 47 CFR 64.1200. Marketing and promotional texts are a different category and require prior express written consent. That is why the templates above do not include offers.
- Do not auto-enroll callers in marketing. Calling your practice is not consent to receive your promotions. Add callers to marketing campaigns only after you collect proper consent.
- Honor opt-outs. If the caller replies STOP, they are out, immediately and automatically.
- Register your texting. Business texting from local numbers in the US requires A2P 10DLC registration, and your registered use case should describe customer care messaging like this. Unregistered traffic is filtered or blocked by carriers.
- Keep health details out of the first text. You do not know for certain who has the phone, so the opening message should not mention a treatment, a procedure, or an appointment. Let the patient tell you what they need. If protected health information will be discussed by text, make sure your platform is set up for HIPAA-compliant use with a signed BAA.
For outbound marketing texts, our SMS compliance checklist and guide to opt-in texting laws cover consent, quiet hours, and disclosures in detail.
Setup Checklist
- Set the ring timeout to 10 to 20 seconds. Long enough for your team to pick up, short enough that the caller is still holding the phone when the text arrives.
- Send from the number they called. A text from an unfamiliar number looks like spam.
- Add a wait step or a filter for repeat callers. One text per caller per window, no matter how many times they try.
- Branch by business hours. Use your hours to decide which template sends.
- Create a callback task. The text buys time; a person still has to follow up. Assign the task to whoever owns new inquiries.
- Tag the source. If you use call tracking numbers per channel, carry the source onto the contact so you can see which campaigns drive calls you miss.
- Use one method, not two. Many phone systems have a built-in missed call toggle as well as workflow-based text back. Running both sends duplicate texts. Pick the workflow, which gives you the controls above.
- Test it. Call from a personal phone during and after hours, let it ring out, and check what arrives.
Common Mistakes to Avoid
- Promotions in the first text. “Sorry we missed you, 20% off filler this week!” turns a compliant reply into a marketing message.
- Texting landlines. Some callers ring from office or home landlines that cannot receive texts. Expect a small share of messages to fail, and keep the callback task as the backstop.
- No human follow-up. The text acknowledges the caller; it does not answer them. If replies sit in the inbox for a day, you have only moved the problem.
- Generic sender identity. Always name the practice in the first line. Callers should never wonder who is texting them.
- Ignoring the data. Track how many calls you miss, how many text backs get a reply, and how fast your team follows up. A high missed-call rate during business hours is a staffing signal, not just an automation opportunity.
Beyond the Text: Answering More Calls
Missed call text back is a safety net. The better outcome is a call that does not get missed. Two options worth considering:
- Ring groups and call routing. Ring several team members at once, or route overflow to a second location, before a call counts as missed.
- A voice AI agent. An AI voice agent can answer the calls your team cannot get to, handle common questions about services, hours, and policies, and text the caller a booking link while they are still on the line. It handles logistics; clinical questions should still route to your team.
How Aesthetix CRM Handles Missed Calls
- Built as a workflow. Missed call text back runs as a workflow automation, so it can branch by business hours, skip repeat callers, personalize with merge fields, and create a callback task for the right person. Our onboarding team typically sets it up as part of your account configuration.
- SMS or WhatsApp. Because it is a workflow, the follow-up can go by text, by WhatsApp, or both.
- One inbox. Replies, the missed call, and any voicemail land on the patient’s record in the conversations inbox, next to every other channel.
- Call tracking and recording. Every call, answered or missed, is logged with its source through call tracking, so you can see where missed calls come from.
- Voice AI. The AI Employee includes voice agents that answer calls your team cannot get to and text the caller a booking link.
Text back messages count toward your normal messaging usage.
Frequently Asked Questions
What is missed call text back?
Missed call text back is an automation that sends a text message to a caller when their call to your business goes unanswered. The text acknowledges the call and invites the caller to continue by text, so a missed call becomes a conversation instead of a lost lead.
Is missed call text back TCPA compliant?
A one-time informational text replying to a call the person placed is generally treated as covered by the caller’s prior express consent. It stays on the safe side as long as it is an acknowledgment and a next step, not a promotion. Marketing texts require prior express written consent, so do not include offers in the text back and do not add callers to marketing campaigns without consent. Have counsel review your setup.
What should a missed call text say?
Name your practice, apologize for missing the call, and give one easy next step: reply by text, or request a time through a booking link. Keep it under two short sentences where you can, leave out promotions, and do not mention a specific treatment until the patient tells you what they need.
How fast should a missed call text back send?
Within seconds of the call being marked missed, while the caller still has their phone in hand. Set your ring timeout to 10 to 20 seconds so calls are classified as missed quickly, and add a short wait step only to prevent duplicate texts to repeat callers.
Does missed call text back work with landlines?
No. Landlines generally cannot receive text messages, so a text back to a landline caller will fail. That is one reason every text back should also create a callback task for a team member.
Can missed call follow-up use WhatsApp instead of SMS?
Yes, if your platform supports it. In Aesthetix CRM, missed call follow-up is a workflow, so it can send by SMS, WhatsApp, or both.
The Takeaway
Every practice misses calls. The difference is whether a missed call ends the conversation or starts one. One well-written, compliant text, sent within seconds, plus a person who follows up promptly, recovers patients who would otherwise have called the practice down the street.
